Why is Square, the biggest RPG publisher with millions of manhours of experience developing RPGs, unable to make a game like Baldur's Gate 3 like Larian, which was a struggling indie developers just a few years ago?

Why is Diablo credited as the first ARPG when Ultima Underworld came out half a decade before it, and is much more ambitious, being 3D and allowing you to jump and swim?

Do any of you guys struggle with storylines in games?

Games that, in my opinion, feature incredible lore driven by environmental storytelling—explaining things never explicitly mentioned in the main narrative.
